چکیده

Carotid artery disease is a major cause of ischemic stroke, the risk of which is directly related to the severity of stenosis and presence of symptoms.1,2 Stroke is the third leading cause of death in the United States, with approximately three quarters of a million strokes per year. Stroke is the leading cause of functional impairment, with more than 20% of survivors requiring institutional care and up to one third having a permanent disability.3 More worrisome, however, is the fact that as the population ages, the number of patients having strokes appears to be increasing.4 The pathophysiology of stroke may be broadly classified as hemorrhagic, embolic, or ischemic. The majority of strokes are caused by embolic events due to atheroemboli from the carotid artery, the ascending aorta, and arch vessels or cardiac thromboembolism from the left atrium or ventricle. It is estimated that carotid artery stenosis is responsible for 15% to 20% of all strokes.5 As percutaneous treatment options expand, there is uncertainty about appropriate therapy for carotid disease. This document will focus on 3 current controversies: (1) carotid artery revascularization in asymptomatic patients, (2) carotid artery stenting (CAS) in patients who are considered to be at increased surgical risk for carotid endarterectomy (CEA), and (3) the current role for CAS in patients at average surgical risk.

Atherosclerotic Peripheral Vascular Disease Symposium II: intervention for renal artery disease.

The primary goal of this American Heart Association renal intervention writing group was to discuss current controversies related to renal interventions and to recommend important areas of clinical research and advocacy initiatives in this peripheral arterial bed. The 4 areas covered in this section include (1) management of asymptomatic renal artery disease, (2) treatment of ischemic nephropat...

Evaluation of Diameter Changes, Stress-strain Elastic Modulus and Stiffness in Normal and Atherosclerotic Common Carotid Arteries in Both Sex Based on End Pressure Variation

Evaluation of elastic properties of major arteries is subject of great interest with respect to the development of vascular diseases. In this study changes in diameter and cross-sectional area, stress-strain elastic modulus and stiffness of the common carotid arteries in healthy and atherosclerotic women and men were evaluated by using indirect end pressure changes. Variations in diameter and c...

Atherosclerotic Peripheral Vascular Disease Symposium II: screening for atherosclerotic vascular diseases: should nationwide programs be instituted?

It has been proposed that vascular screening programs should be widely established to provide earlier detection of peripheral artery disease, carotid artery disease, renal artery disease, and abdominal aortic aneurysms (AAAs) to diminish the societal burden of these illnesses. Early detection of these conditions could lead to treatments that offer the potential to reduce the incidence of fatal ...

Controversial treatment of atherosclerotic renal vascular disease: the cardiovascular outcomes in renal atherosclerotic lesions trial.

Atherosclerotic renal artery stenosis (RAS) is a relatively common problem, affecting from 1% to 5% of patients with hypertension.1,2 Given the high prevalence of hypertension, it follows that there are from 2 million to 4 million individuals with RAS in the United States alone. Autopsy data demonstrate that the incidence of RAS increases with age affecting 18% of individuals between the ages 6...
